RESUMO

Background: Gensini (G score) is one of the most widely used scoring systems in cardiology. It is an objective method to determine the coronary artery disease severity according to angiographic findings. The aim of this work was to assess the relation between G score and the chronicity of diabetes mellitus (DM) in cases undergoing coronary angiography. Methods: This prospective cohort research was carried out on 300 cases with diabetes mellitus (DM) on antidiabetic treatment who were referred to diagnostic coronary angiography for suspected coronary artery disease. G score was calculated for measurement of the coronary artery disease severity. During the period from October 2020 to Aril 2022 Results: Age, smoking, chronicity of DM, cholesterol, triglycerides test, and electrocardiogram were statistically prominent positively related with G score, while female gender was statistically significant negative relation with it. Also. Mean G score was statistically prominent higher in cases with ischemic changes than in cases without ischemic changes. Therefore, the chronicity of DM was statistically significant predictor of G score. Conclusions: significant positive relation between the G score and the chronicity of DM.

Background: The Symptomatic manifestation of pediatric arrhythmias varies according to the age. Patients with disturbances in cardiac rhythm have several complaints, but also could be totally asymptomatic. This study aimed to assess clinical characteristics of Tachy and Brady arrhythmia in neonates, infant, children and adolescents of various age groups. Methods: This cross-sectional study included 253 patients of pediatric age below 18 years old presented with different types of tachyarrhythmia or bradyarrhythmia. All patients were subjected to clinical general examination of patient, vital data collection, local cardiac examination, 12 leads electrocardiography, ambulatory ECG Holter monitoring, exercise ECG stress testing if indicated and transthoracic echocardiography. Results: There was a significant relation between age of presentation of the study patients and classification of arrhythmia (P < 0.001). Palpitations was the most common presentations of arrhythmia in our study patients (32%) followed by dyspnea in 17.8 % of the patients. 20.6 % of the study patients with arrhythmias were asymptomatic. Congenital heart anomalies were in 43 % of the patients. Conclusion: Pediatric arrhythmia can be totally asymptomatic and discovered accidentally. Congenital cardiac anomalies and cardiac arrhythmias are closely correlated. Holter monitoring and exercise ECG testing can help in diagnosing pediatric arrhythmia. Untreated or persistent arrhythmia causes LV myopathy and dilatation. Antiarrhythmic drugs can have their harmful effect on the growing child.

Background: Coronary artery disease (CAD) early diagnosis remains a clinical problem in patients with non-ST elevation acute coronary syndrome (NSTE-ACS), especially in the regional wall motion abnormalities’ absence on presentation by Echo. This study assessed the relationship between ECG changes and speckle Tracking in subjects with acute NSTE-ACS. Methods: This prospective trial was performed on 100 subjects with NSTE-ACS. All subjects were subjected to laboratory tests [complete blood picture, renal function test, liver test profile, lipid profile, RBS and cardiac biomarkers (cardiac troponin, creatinine kinase and CK-MB)], 12 lead ECG, Echocardiography (TTE, speckle tracking), Image analysis and coronary angiography. Results: regarding IVRT, TDI e`, TDI a`, a considerable difference between the two groups were found. DBP was considerably lower in STD group in comparison with TWI group (P value= 0.047). IVRT, TDI a` and GLS were considerably higher in STD group in comparison with TWI group (P value= 0.024, 0.031, 0.003 respectively). Conclusions: Speckle tracking could be used as part of standard echo for the examination of individuals suffering from NSTE-ACS.
